Pricing & Payment
The short answer – to provide better care! Our treatment pricing is focused on providing value for your money. Our personalized, hour-long sessions typically allow our providers to accomplish treatment goals in less sessions when compared to a standard outpatient therapy setting where the provider often sees 2-4 patients/hour. In addition, our flexible direct-pay model allows patients to seek treatment or have a consultation without having to get a prescription for physical therapy from your doctor. This decreases wait time for treatment and allows our patients to seek the care they need directly.
Most patients with insurance coverage have high-deductible health plans with deductibles of $2,500 or more. The average physical therapy clinic bills insurance $250 per session, and would be the patient’s responsibility until they meet their deductible. Our direct-pay method may actually lower your out of pocket costs for physical therapy treatment.
We accept HSA & FSA cards, all major credit cards, cash, or check.

Payment for all treatment is due at the time of service. If desired, our providers will provide you with an invoice (super bill) that can be submitted to your insurance. Most private insurance plans offer “out-of-network” reimbursement or deductibles. While we cannot guarantee reimbursement by your insurance provider, we will provide you with the resources necessary to request reimbursement for our services.
Please note, if you are a Medicare beneficiary, you can only seek private pay physical therapy treatment for services not covered by Medicare. You cannot submit our superbills for reimbursement to either Medicare or secondary insurance. If you are looking for Medicare-covered services, we would be happy to refer you to a Medicare-enrolled provider.
Fit Family Physical Therapy visits may be treated as “out-of-network” physical therapy treatments by insurance plans that offer those benefits. Our providers will give you an invoice (or super bill) that can be submitted to your insurance with all of the necessary information (license numbers, tax ID, business name, etc.) as well as your specific diagnosis and billing codes. You may choose to submit bills after each visit or any other frequency, typically up to one year following the provided treatment visit.
While we cannot guarantee reimbursement by your insurance provider, we will provide you with the resources necessary to request reimbursement for our services.

No! Our direct pay method allows our patients to seek care when they need it without going through their doctor for a prescription for physical therapy.
If you have a Medicare Insurance Plan, you may only seek physical therapy treatment through a direct-pay, direct access practice like Fit Family Physical Therapy for services not covered by medicare such as prevention, wellness, and fitness. Fit Family Physical Therapy does not have a relationship with Medicare, and thus does not bill Medicare for any reason.
Treatment & Visits
The full treatment session is spent with one of our doctors. No techs, assistants, double-bookings. Our standard treatment sessions are 60 minutes.
As with any type of health treatment, it depends! Some patients need to be seen twice weekly or weekly for 4-6 weeks, while others may only need to be seen monthly. However, our personalized treatment plan and full hour-long sessions will ensure you see longer-lasting results with fewer sessions. During your initial evaluation or discussion, your Fit Family physical therapist can give you more insight into how often and how many visits are recommended.
See above for number of visits. If you are unsure which option to purchase, many patients opt to start with an individual session. After thorough discussion with your physical therapist regarding plan of care and treatment length, you can then choose to purchase package options if they better suit your physical therapy treatment plan needs.
No, you do not need to purchase any equipment for treatment sessions. The physical therapist will bring all of the equipment they need to the visit. Your physical therapist may recommend equipment for use at-home for exercises outside of treatment sessions, however it is never required. Your provider will create the treatment plan with your available equipment in mind.
For personal training clients, your personal trainer will discuss your fitness goals and the most appropriate place to meet to achieve those goals with the correct equipment.
At your first visit, your physical therapist will discuss your physical therapy concerns and medical history with you, perform tests and measures to assess physical function, and discuss a recommended plan of care for continued physical therapy treatment based on your goals and desired timeline.
At any subsequent visits, your sessions will follow the treatment plan agreed upon by you and your physical therapist. Most visits include a component of manual therapy (hands-on, direct care), guided progression of exercise specific to your condition, and a review of your home exercise plan and any questions you may have. Your physical therapist will work directly with you for the entire session.

Yes! At the end of each visit, your provider will schedule your next visit(s) to continue your plan of care. You may choose to schedule one or more future visits.
If you are scheduling your first visit or a visit for a new physical therapy concern, we do not recommend scheduling multiple visits until after your initial evaluation is completed. During this initial evaluation or discussion, your Fit Family physical therapist can give you more insight into how often and how many visits are recommended. If you need to cancel an appointment, you can do so in our PT Everywhere app or by calling our team at 480-788-0046 to cancel. Any appointments within 24 hours cannot be cancelled within the app and you will need to call us to inform us of the cancellation or request to reschedule.
Please be advised that cancellations made after 8AM on the day of the appointment will be subject to the full visit charge. This includes appointments where our provider is unable to access the property, is turned away, or the client is unavailable.
